Description:
The Participant/Subject Contact Information CRF Module records the contact information for the study participant/subject. The template form also has a place to record the contact information for persons who regularly interact with the subject. The contact information data are not part of the clinical data management system so as to protect the identity of study participants, but rather are kept in a locked location for authorized study personnel to reference.
