Description: 15 questions, plus 6 disability due to migraine questions, plus 9 medical history questions
Comment: An optional measure for pediatric epilepsy (i.e., younger than 12 years of age). This exam does not have the same degree of validation as the other two recommended instruments.
Primary Dependent Measures: Migraine, Migraine with aura, Migraine without aura
Time Estimates: ~15 minutes.
