| Short Description of Instrument |
The Risk Factor Questionnaire was developed by the PD-RFQ Epidemiology Working Group of the Collaborative Centers for Parkinson's Disease Environmental Research. This validated questionnaire was developed to provide a standard assessment tool for general use in epidemiologic studies of Parkinson's disease. It has been validated for self-report and interview. It is available in 13 languages. A universal database has been developed in the free software, Epi-Info, available from the CDC website.
The PD-RFQ assesses exposures in the following domains: caffeine, tobacco, alcohol, physical activity, head injury, residential and occupational histories, NSAID and hormonal medications, body habitus and pesticide exposure.
